
What Is an mRNA Vaccine and Which Brands Are Available?
An mRNA vaccine (messenger Ribonucleic Acid) is a newer type of vaccine that does not use the whole virus and does not cause an actual infection. Instead, it uses mRNA genetic material to instruct the body’s cells to produce a specific part of the virus. In the case of COVID-19 vaccines, the mRNA carries a replicated genetic code for a specific part of the Spike Protein, an important structure on the virus’s surface. After vaccination, the cells temporarily produce a harmless Spike Protein so that the immune system can learn to recognize it as a foreign substance. This stimulates antibody production and increases T-cell activity, preparing the body to fight the virus if an actual infection occurs in the future.
Currently, two mRNA vaccine brands have been approved and are widely used in many countries, including Thailand:
- Pfizer vaccine, manufactured by BioNTech/Pfizer in the United States, has an overall efficacy of 95% in preventing symptomatic COVID-19 after completion of two doses.
- Moderna vaccine, manufactured by Moderna in the United States, has an overall efficacy of 94.1% in preventing symptomatic COVID-19 after completion of two doses.

After receiving an mRNA COVID-19 vaccine, some side effects may occur. Most are temporary and resolve on their own within 2–3 days after vaccination. Common symptoms include:
- Pain, swelling, or redness at the injection site.
- A mild to moderate fever, chills, or general body aches.
- Headache and fatigue.
- Nausea and vomiting.
- Muscle or joint pain.

- Myocarditis and Pericarditis: According to United States health insurance claims data from 2023–2024 (2), the incidence was approximately 8 cases per 1 million vaccine doses among people aged 6 months to 64 years. A higher rate was identified among males aged 16–25 years within 7 days of vaccination, at approximately 38 cases per 1 million doses. Following these data, the FDA directed Pfizer/BioNTech and Moderna to expand the warning language concerning the risk of myocarditis and pericarditis to improve transparency when presenting risk information to the public.
- Severe allergic reaction after vaccination, or Anaphylaxis: The reported incidence was approximately 11 cases per 1 million vaccine doses (3). Abnormal symptoms may occur within 15–30 minutes after vaccination, including hives, an itchy rash across the body, difficulty breathing or wheezing, chest tightness, swelling of the lips, tongue, or throat, an abnormally rapid pulse, nausea, and vomiting. The condition can be treated with epinephrine and standard medical care to help the patient recover normally.
Nevertheless, reports from the United States Centers for Disease Control and Prevention (CDC) and the World Health Organization (WHO) state that mRNA COVID-19 vaccines continue to provide benefits in reducing disease severity and mortality compared with the risk of rare side effects.
